Do you take insurance?
I do not bill insurance. As a solo business owner, I do not have the capacity to bill insurance. I accept HSA/FSA. I can provide a superbill upon request for seeking potential reimbursement from your insurance. Please contact your insurance if Superbills applies to you and take note of the requirements needed to submit it.
NOTE: I am not responsible if your insurance denies reimbursement and I will not provide a refund or negotiate with your insurance company. Reimbursements are not guaranteed so it is important you have all the requirements needed from your insurance.
What is a Superbill?
A superbill is a detailed invoice/receipt that lists the services provided, dates, and fees.
It includes information like the therapist's name, NPI number, address, and contact information.
It also includes patient information, such as name, date of birth, and insurance details.
It typically includes CPT codes (Current Procedural Terminology) for the specific massage techniques performed and ICD codes (International Classification of Diseases) for the conditions being treated.

What is my coverage for massage and how many hours/units? (units are 15min)
Do I need a doctor’s referral or prescription?

Any other special requirements to receive coverage?
What is my out-of-network co-pay or co-insurance?
What is my deductible for the year?
Has my deductible been met for the year?
Office Policies
Cancellation/No-Show:
Please provide a 48 hour notice if you need to cancel or reschedule. You will be notified 2 days before your appointment to allow ample time to cancel or reschedule to avoid any fees; and an hour notice for friendly reminder before your session.
Cancellations made under 48 hours are charged 50% of the service. No-Shows and late cancelations under 24 hours are charged 100% of the service price which will be billed to the credit card on file.
To avoid being charged a fee, you may find someone to take your place for the appointment.
If I am able to fill the appointment time, I will void the fee or apply any charges as a credit to your next session.
I appreciate your cooperation as this allows me to successfully operate my business and allows me to find a person in need of care to be seen.
Unusual Circumstances:
If you are feeling sick, or are exhibiting symptoms of any kind, please notify me immediately. Waiting until the last minute will result in a late cancellation should you be unable to keep your session time.
Cancellations under 48 hours due to illness still result in a cancellation fee.
To avoid being charged a fee, you may find someone to take your place for the appointment.
If I am able to fill the appointment time, I will void the fee or apply any charges as a credit to your next session.
Please take care of your wellbeing and consider my own health as well. You may be turned away and be charged for a cancellation fee if you are presenting symptoms.
Payment Policy:
All appointments are booked online with a card held on file. This is not pre-payment. Your service fee is due upon service date.
Acceptable Payments are:
Debit, Credit, HSA/FSA, Cash, or Venmo
For payments over $200, payments are appreciated when done directly through Venmo or Cash
If you choose to use your FSA or HSA card, you cannot leave gratuity with that account, and a massage prescription should be kept on file. .
